Format:
1. Acknowledge what they have done well (1 line, sincere)
2. State the issue (specific, with date / ticket / order ID)
3. The SLA breached (quote the contract)
4. The impact (with numbers)
5. What you want (specific remedy + deadline)
6. The alternative (escalation path or contract review)
7. Door open for relationship if they fix it
